HOW TO USE THIS TEMPLATE:
Make sure you are in the regular editor (not the Rich Editor. Copy(CTRL-C) everything below the first set of --'s and return to the Edit version of your notebook page; Make sure you are in the Regular editor again, and paste the copied text into the page. Return to the Rich Editor View and add in all of your information.
